The Next Step – Moving In

Now that you have decided which of our AssistCare Communities of Amador County is right for you, it’s time to complete our pre-admission packet. The Pre-Admission questionnaire is a Self-Assessment guide published by the California Department of Social Services. This will help us better define our new residents care needs. Next the physician’s report needs to be completed by your doctor and a copy given to us. A current TB test result needs to supplied as well.

The cost of your stay is determined by the community, the size of the room and the level of care that is determined to be appropriate for you. We have structured our levels of care to include a broad range of services. Our intent is to make our rates as simple and inclusive as possible while retaining the flexibility to tailor individualized care plans with the lowest possible cost.
